The Phoenix Recovery Center (TPRC) provides rehab with a full continuum care for residents of the Wasatch Front from Ogden to Provo, including the Salt Lake City metropolitan area. TPRC is Joint Commission accredited, LegitScript certified, ranked 5th in Utah for addiction treatment, and one of the 2026 Salt Lake City Titan award winners.
We provide dual-diagnosis treatment (rehab for people recovering from co-occurring mental health and substance abuse disorder) for clients in Utah and across the U.S. We also offer mental health and addiction in separate treatment tracks depending on each patient’s needs.

Flying in From Out of State
For out-of-state patients, we can coordinate travel & transportation to and from the Salt Lake City Airport. Please call our admissions coordinators at (801) 438-3185 to make travel arrangements.
Driving From the North (including Salt Lake City to Ogden)
Head south on highway I-15 S and take exit 293, turning right onto UT-151 W/South Jordan Pkwy. Immediately after turning right onto UT-151 W/South Jordan Pkwy, take a left onto Jordan Gateway. Take the next left onto W Cardon Ridge Way and park in the lot south of our building at 489 S Jordan Pkwy Suite 400, South Jordan, UT 84095.
Driving From the South (including Lehi, Orem, Provo, Saratoga Springs, and Eagle Mountain, and Provo)
Head North on I-15 N and get off at exit 293. Turn left onto UT-151 W/South Jordan Pkwy. Immediately after, take a left onto Jordan Gateway. Take the next left onto W Cardon Ridge Way and park in the lot south of our building at 489 S Jordan Pkwy Suite 400, South Jordan, UT 84095.
Driving From the East (including Sandy and Draper)
From Sandy, head south to get on E 10600 S. From Draper, head north to get on E 10600 S. Once on E 10600 S, the street will change names two times, but just continue driving West. At S State Street, the street name changes to W 10600 S and after passing I-15, the street name will be South Jordan Pkwy.
Once you pass I-15, take the a left at the first available turn at a stoplight onto S Jordan Gateway. Take the next immediate right onto W Cardon Ridge Way, parking in the lot south of our building at 489 S Jordan Pkwy Suite 400, South Jordan, UT 84095.
Driving From the West (including Tooele, Grantsville, and Skull Valley)
From Tooele, head north on UT-36 N/N Main St until you can merge onto I-80 E via the ramp to Salt Lake City. From Grantsville and Skull Valley, merge straight onto I-80 E at your nearest on ramp. After you pass I-215, use any lane to merge onto I-15 S/I-80 E toward Cheyenne/Las Vegas. Get off the interstate at exit 293 in South Jordan. Turn left onto UT-151 W/South Jordan Pkwy. Immediately after, take a left onto Jordan Gateway. Take the next left onto W Cardon Ridge Way and park in the lot south of our building at 489 S Jordan Pkwy Suite 400, South Jordan, UT 84095.
